I can see represented participant directly and we feel some sort of involvement. This picture gives some awareness about smoking. And if viewer is a smoker, they will feel uncomfortable and think about quitting the smoking and their health.

I can see represented participant indirectly and we feel some sort of detachment between our own and represented participant in this picture. This picture does not give a strong message to viewer. I know that what want to tell to us but it is not powerful and does not lead to our participation.
I like the PICTURE.... nice one... nice advertising as well.... clever way to send the message... love it.
